Pay Stub Template Checklist: Fields, Examples and Review Steps

A useful pay stub template clearly separates employer and worker details, dates, earnings, employee taxes, other deductions, net pay and any year-to-date totals. Choose a layout that supports the actual payroll arrangement and applicable statement requirements. A blank template cannot determine the correct figures for you.

This guide provides a field checklist and examples to review. It does not include a downloadable blank template or promise that one layout meets every state’s rules.

Pay stub template field checklist

These are practical review categories, not a complete legal field list for every jurisdiction. Check state wage-statement requirements before choosing a final format.

Hourly and salary templates serve different records

For hourly payroll, a layout should make hours, rates and earnings easy to follow. A single unlabeled earnings box can obscure how the amount was calculated. For salary payroll, identify the salary earnings for the relevant period and distinguish additional compensation. Salary status alone does not resolve overtime eligibility.

Compare hourly pay stub calculations and salary pay stub calculations with the available fields.

Illustrative layout review

Suppose a fictional record has $1,000 gross pay, $150 in illustrative employee taxes and $50 in other deductions. The net-pay field should show $800. Labels should explain which amount belongs to which category. These figures are an arithmetic example, not a tax calculation or a completed payroll record.

See pay stub examples for additional scenarios. Do not copy an example’s deductions into an actual statement.

Printable does not automatically mean complete

A statement that looks clear on screen may print with missing columns, clipped figures or unreadable labels. Inspect the finished document, including every page. Check that names, dates and totals remain legible and that current-period and cumulative amounts cannot be confused.

Protect sensitive information

Use only the identifying information needed for the record and applicable requirements. Do not add a full sensitive identifier merely because a generic template provides a box. Keep completed payroll documents in an appropriately protected location and share them only through suitable channels.

Before you generate a document

  1. Gather accurate source records.
  2. Check the selected format against the payroll arrangement.
  3. Reconcile earnings, deductions and net pay.
  4. Verify YTD figures against the previous valid record.
  5. Inspect the finished document for omitted or clipped information.

Use the payroll review checklist and the PayStub Express creation guide, then open the generator. Confirm the available fields and document options in the app before completing your record.
